New Online Forms Now Available

No printing. No scanning.

As part of our strategy to help modernize some aspects of your Union and to make things easier and more accessible for our Membership, the SIU Canada has transitioned many of our commonly used forms from PDF downloads to secure online web forms available directly on the SIU Canada website.

Instead of printing, filling out, and mailing or emailing PDF forms, Members can now complete and submit applications and claim forms directly through our website.

What’s changed

The following forms are now available online:

  • Applications for admission and membership documents
  • Medical and dental claim forms through the Seafarers’ Medical Plan
  • Other frequently used Member forms and requests

Why this change matters

  • Faster submission and processing
  • No printing or scanning required
  • Forms can be completed on any device, including phones and tablets
  • Clear prompts help reduce errors and missing information

All submissions are sent securely and reviewed by the appropriate Union department or the Seafarers’ Medical Plan team.

Members who prefer or require PDF versions of any form can still request one by contacting the appropriate department directly.
